Jurisdiction note before you use the template
Do not treat OSHA and CDM as one combined framework. They apply in different jurisdictions.
- In the United States, construction safety requirements are set out in OSHA 29 CFR Part 1926. OSHA also explains how workplace inspections are conducted in its inspection process guidance.
- In Great Britain, construction project duties are set out in the Construction (Design and Management) Regulations 2015. The HSE also publishes CDM 2015 guidance for clients, designers, principal designers, principal contractors, contractors, and workers.
If your project is in another jurisdiction, keep the reporting structure but replace the legal references with the governing local requirements. The template helps you organise evidence; it does not decide legal compliance.

Severity codes
For the app workflow, keep severity to the supported values: Critical, Minor, and Positive. If your company uses extra internal labels, keep them in your own procedure rather than presenting them as supported app values.

Photo evidence rules
Photos make a construction inspection report easier to review because they connect the written finding to a visible site condition. Keep the process simple:
- Take the photo when the finding is recorded, not later from memory.
- Capture one wide shot for location and one close-up for the condition.
- Number photos in the same order as the findings list.
- Put the photo reference in the finding row.
- Avoid mixing unrelated issues in one photo reference unless the report clearly explains each issue.
For example, if finding 4 is "missing mid-rail on east elevation scaffold", photo 4 should show that location and condition. The report reader should not have to search through a general site photo set.
