How to fill the finding log

Each row in the finding log is one observation. Keep the condition factual and separate it from the risk interpretation.

Severity definitions keep the log consistent across inspectors:

  • Critical: immediate danger; work must stop or the hazard must be controlled before continuing
  • Major: significant risk; correction needed within a defined short window
  • Minor: low-risk deviation; correct on a normal schedule
  • Positive: a control that is working well and should be maintained

OSHA and eCFR references

Most OSHA standards do not prescribe one universal inspection report format. They do create duties that a site safety report can help evidence: hazard identification, worker protection, corrective action, and recordkeeping. Applicability depends on the activity, employer, jurisdiction, and whether the work is construction or general industry.

For construction examples, use construction-specific references such as:

  • 29 CFR 1926.20(b) — accident prevention responsibilities
  • 29 CFR 1926.21(b) — safety training and education
  • 29 CFR 1926.95 — criteria for personal protective equipment
  • 29 CFR 1926 Subpart M — fall protection
